Your First Visit
With your first visit, you are about to undertake an exciting step toward
better health. Chiropractic care is different than traditional medical
care, and you will begin to learn what those differences mean in terms
of moving toward better health
In order to provide you with the very best of care, you will be asked
to fill out general contact information, along with insurance information
if applicable, consent form,a complete health history form and sheets
that ask you about where your specific pain is located in your body.
If possible, these sheets would be preferred to be filled out in advance (to speed the first visit process)

Depending on the doctor you meet with, they will describe to you what
chiropractic care is, and entertain any questions, comments or concerns you
may have. We are committed to making sure our new patients understand
as much about the care they will receive here so you can make informed
decisions and more actively participate in your health care.
